GENERAL
Do you as the parent or guardian of a youth accept the following disclaimer, waiver and consent for entering a minor into the Central Manitoulin March Break Youth Fishing Tournament 2023?
When participating in or entering yourself or a youth in your care into the Central Manitoulin March Break Youth Fishing Tournament 2023 you do so at your own risk and those under your cares risk. The Municipality of Central Manitoulin its partners and tournament sponsors do not accept liability for any claims including the following specific items for participants, contest entrants and their guardians while participating in this tournament that result in property damage, personal injury or loss of life.
Central Manitoulin March Break Youth Fishing Tournament 2023, it’s sponsors and affiliates are not liable for dangerous ice and water conditions resulting in personal injury loss of life or property damage. Nor are they liable for travel to, from or during the tournament resulting in personal injury loss of life or property damage. Check with local authorities on ice conditions before proceeding onto a frozen lake.
The participants parent or legal guardian agreeing to this liability waiver document assume responsibility for the participant participating even without the legal guardian or with another guardian; and that the parent or legal guardian agreeing to this waiver is responsible for relaying the tournament information and the waiver information, informing the other parties of their liability and the information contained within the tournament documentation.
Participants, entrants and their guardians must follow all local, provincial and federal laws. Tournament organizers are not liable for any fines, court proceedings, court or lawyer costs incurred, confiscation of property or jail terms resulting from participation in the tournament.
Participants, entrants and their guardians must be fishing with a valid Ontario fishing licence or under the age required for a licence and following Ontario Fishing Regulation and Rules Summary, abiding by the Regulations of the Fish and Wildlife Conservation Act, the Fisheries Act, The Department of Transport, Highway Traffic Act, the Bait Management Zone regulations, the Invasive Species Act Regulations or any other acts or regulations that pertain to traveling to and from the participation in the tournament, the participation in the tournament itself and participating in the act of fishing; and the Rules and Regulations of the Central Manitoulin March Break Youth Fishing Tournament 2023.
WAIVER
I, the stated parent/guardian of the registrant, a minor, agree that the registrant and I will abide by the rules of the Central Manitoulin March Break Youth Fishing Tournament, the Municipality of Central Manitoulin its affiliate organizations and sponsors, and understand that he/she/I in attending any event(s)/program(s) at the in the tournament or locations described in the contest boundaries or any location(s)/facility(s) does at his/her and guardians own risk.
The Central Manitoulin March Break Youth Fishing Tournament and its owners, employees, sponsors, volunteers and agents, shall not be liable for any damage whatsoever arising from any personal injury or property loss sustained by participant and his/her/my family in or about any event(s)/program(s) in the described premises.
In registering for the event(s)/program(s), I recognize, acknowledge and assume full responsibility for all injuries, damages or loss which may occur in or about any event(s)/program(s) on the premises and he/she/I hereby fully release, discharge and otherwise indemnify and hold harmless the Central Manitoulin March Break Youth Fishing Tournament, all associated facilities, organizations, and its owners, employees, sponsors, volunteers and agents from any and all claims, demands, damages, loss, rights of action, present or future resulting from or arising out of any person’s participation in any event(s)/program(s), in any manner, in any and all activities connected with or associated with this event(s) or use of its facilities.
In addition, he/she/I fully agree(s) to follow the rules of play and conduct set by the Central Manitoulin March Break Youth Fishing Tournament. He/she/I understand(s) that failure to do so may result in suspension from participation. I hereby waive and release all claims for injuries, damages or loss arising out of this program that I or the above participant might sustain. The terms “I”, “me” and “my” refer to Parent(s) Grandparent(s), Great grandparent(s) or otherwise known as the legal guardian(s) as well as the participants in this event.
CONSENT
As the participant/parent or legal guardian consent to the above listed participant to be entered into the tournament based on the above accepted liability disclaimer and waiver. I hereby authorize the Central Manitoulin March Break Youth Fishing Tournament 2023 and its assigns to utilize any and all photographs, pictures, or other likeness of legal parent/guardian and the registrant as they deem appropriate in its promotional materials.
